Required Forms for Auditions: Music Scholarship Application Form (PDF format, 137k) You will need to bring this completed form to your audition. Music Recommendation Form (PDF format, 237k) You will need to ask two people to complete this form and mail or fax it to us. These files are in PDF format, so you will need the Adobe Acrobat Reader to view and print it. NOTE: You can fill out the application form on your computer (using Acrobat Reader), then print it out.
