Student Activities

Mission Statement

The purpose of the Office of Student Activities is to involve students with the institution. Students' involvement includes the opportunity to participate in co-curricular activities, community services and campus government; develop leadership skills; and positive interpersonal relationship skills and be exposed to a variety of cultures and ideas. Student Activities supports the institutional mission by promoting retention and success in college and by promoting opportunities for cultural enrichment and the development of good citizenship through activities and services.

Student Activities Forms

All chartered organizations are required to complete the following forms at the beginning of the academic year:

  • Student Organization Active Status Application
  • Student Organization Registration Form

Organizations not completing these forms will be determined inactive and will not be able to hold meetings or events.

The following forms must be completed prior to any campus event or purchase of organizational supplies:

  • Student Activities Proposal Form (This form is for any on or off campus events, meetings, community service, travel, etc.)
  • Requisition Request Form (This form is for all organization supply purchases. It will only encumber the money. An invoice and check request will still be needed for processing payment. This form does not have to be completed for reimbursements.)

The Student Activities Proposal Form must be submitted at least two weeks in advance of the event. The Requisition Request Form must be submitted at least three weeks in advance if you prefer that Student Activities complete the requisition. Not submitting these in advance will result in delay of funds or cancellation of meeting/events.

For all off campus events, students must complete the Student Activities Liability Form. This form only needs to be completed once each academic year. For a list of students who have completed the form, please see Current Liability Forms.
